Please arrive at Grace Clinic with the following information:
- A picture ID
- Your insurance card (if you have one)
- Social Security number
- The Social Security number and birthday of any responsible party (spouse, parent or child)
- Employer information for the patient and responsible party
- A complete list of the medications and dosages the patient is currently taking

| Also helpful are copies of your medical records, including test results related to your current medical concerns. Sometimes previous clinical studies don't provide useful information so Grace Clinic physicians may find it necessary to order new tests to ensure the most accurate diagnosis possible. |
